Undergrad Institution: State School
Major(s): Mathematics
Minor(s): Computer Science
GPA: 3.25(Overall)/3.85(Math)
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 165 (89%)
V: 152 (56%)
W: 4.0 (60%)
GRE Subject Test in Mathematics:
M: (<25%) No idea how I scored so low, let's hope it doesn't affect my apps too much.

Program Applying: Pure Math(Combinatorics/Number Theory)

Research Experience: First Research Experience, funded by a grant a research grant from school. Lead to Sole-Authored Publication. Paper has been accepted into Discussiones Mathematicae: Graph Theory. I was then accepted into the McNair's Scholars Program and worked on a different research problem. This lead to a co-authored publication into a very good journal, European Journal of Combinatorics. The result obtained is largely my contribution and I expect my mentor will state so in his letter. Paper is accepted and scheduled to be published soon. Next, I modified the idea from the previous paper and applied it to a different problem. I have since written up a sole-authored paper and it is under review at the journal Integers. I have given 2 talks at a Seminar on my results for the accepted papers, 2 poster presentations, and another talk as required by McNair.
Awards/Honors/Recognitions: Received largest departmental scholarships for my last year. All my research was funded, so that also will look good.
Pertinent Activities or Jobs: On-Campus tutor for two years.
Any Miscellaneous Points that Might Help: Two of my recommenders are UCSD Alums, and one is well-known at the school for being a great researcher. I also have had the opportunity to correspond with a professor at MIT whose research is in exactly the same area as mine has been. My co-authored paper generalizes a result from an emeritus professor at UC Berkeley.
Any Other Info That Shows Up On Your App and Might Matter:
I applied for the NSF Graduate Fellowship, which looks good on the application.

Undergrad Institution: Big State School in the Northeast
Major(s): Math, applied concentration
GPA: 3.03
Math GPA: 3.53
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 166 (91%)
V: 162 (91%)
W: 6.0 (99%)
GRE Subject Test in Mathematics:
M: 470 (8%) Only submitting to UC Davis.
(Yeah, I bombed it. Haven't taken abstract algebra or analysis, so that didn't help)

Program Applying: Masters Programs in Applied and Pure Math

Research Experience: None
Awards/Honors/Recognitions: Merit scholarship
Pertinent Activities or Jobs: None
Any Miscellaneous Points that Might Help: I'm rather pessimistic given my GPA. I am hoping to have decent letters of recommendation. My math modeling professor's letter should be strong; I completed a modeling project on HIV immunology for his class which he was impressed with. I highlighted this project in my personal statement, saying how it inspired me to pursue a graduate level math.

Also important to point out: I was only a math major for 2.5 years. I was a biochem major for the first 2.5 years, and I was a terrible student (Ds and Fs in several classes). After switching to math, my semester GPAs trended upward: 3.0, 3.65, 3.70, 3.9, 4.0. Also, I was only a part-time student as a math major, so I have not taken as many math courses as most other math majors. (I will be taking real analysis this spring, even though I already graduated. Have not taken abstract algebra.)

Applying to:
UC Davis - MS Appl Math - Pending
UC Santa Cruz - MS Sci Comp and Appl Math - Accepted 3/15 No funding :(
NCSU-MS Biomath -Pending
UMass Amherst - MS Appl Math- Accepted 3/9! $1300/semester scholarship.
Wake Forest - MA Math Waitlisted 3/2. Got an email two days later saying they "plan" to accept me.
Western Washington - MS Math-Accepted 3/7! 12k/year TA-ship plus partial tuition waiver 3/9!
San Francisco State - MS Math- Pending

Pertinent Activities or Jobs: TA for a biomathematics course in the biology department as a sophomore
Any Miscellaneous Points that Might Help: Female, 3 grad courses before applications submitted and 1 in final semester, including a year of graduate-level analysis in progress, cohesive statement of purpose
Any Other Info That Shows Up On Your App and Might Matter: 2 letters of recommendation from the math profs I worked with in REUs and 1 letter from the biology prof I work with at my home university. I was warned that some people might look down upon the biology prof’s letter but I know that it will be the strongest and didn’t care.

Applying to Where: (Roughly in order of interest at the time of applying)
Cornell - Applied Mathematics - Waitlisted 1/31

V: 156 (73%)
W: 5.5 (98%)
GRE Subject Test in Mathematics:
M: 600 (37%)

TOEFL Score: (113 = R29/L28/S26/W30)

Pertinent Activities or Jobs: TA for Calculus I, Proofs Course (x2), Analysis I (x2). TA experience for physics and economics courses as well.
Any Miscellaneous Points that Might Help: None.
Any Other Info That Shows Up On Your App and Might Matter: For a variety of reasons highlighted in my SOP, I'm a late comer to mathematics. To make up for my lack of courses, I did extensive self-study with math adviser in such topics as linear algebra (second course), abstract algebra (groups, rings and a bit of modules), complex analysis (up till residue calculus), analysis n R^n, and (a bit of) measure theory. I think I ended up getting a strong letter from my math adviser, as conveyed to me by the DGS at Arizona State. The other letters were from physics professors.

Minor(s): None
GPA: 3.5 (3.9 in Math)
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 168 (95%)
V: 150 (47%)
W: 4.5 (82%)
GRE Subject Test in Mathematics:
M: 590 (34%) - Only submitting to schools that require it. However, I was told this is a good score for people from my school.

Program Applying: Ph.D. in Pure Math

Research Experience: I have done 2 REUs, one in combinatorics and the other in Inverse Semigroup Theory. We are in the process of preparing a paper for the last one and I submitted a working document to most schools. My first taste of research was a project in geometry my freshman year. This past summer, I also did funded independent research in combinatorics at my school. This past semester I did my Capstone research in extending the Inverse Semigroup research I did at my REU.

Awards/Honors/Recognitions: I have made dean's list a few times, made honor roll for my school's athletic conference as well. For the past two years I have been vice president and then president of my school's math honor society.

Pertinent Activities or Jobs: Was a private tutor for the first few years of undergrad. This past year I have been tutoring in the math department and TA'ing Calc II.

Any Miscellaneous Points that Might Help: I have a recommender who graduated from one of the schools I applied to. I have also met and had a good conversation with professors that are on the graduate admissions committee of some of the schools. They all told me they would make sure they read my application thoroughly. Hopefully that helps a bit.

Any Other Info That Shows Up On Your App and Might Matter: I originally was a secondary education minor, so I have experience teaching in a high school and writing lesson plans.

Applying to Where: Note: I applied to the Ph.D. program in Pure Math for all of these schools

Undergrad Institution: Small "top-30" liberal arts, not known for math
Undergrad Major(s): Mathematics/Economics
Concentration: Sci Comp
Undergrad GPA: 3.6ish overall/3.8ish math (3.9+ upper div math)

Grad Institution: Small unranked state school
Major: Mathematics (MS)
Grad GPA: 4.0
Type of Student: DWM

GRE Revised General Test:
Q: 167
V: 168
W: 4.5
GRE Subject Test in Mathematics:
M: 640

Program Applying: Pure Math PhD, probably focusing in scientific computing/numerical analysis but also interested in computational/analytic number theory

Research Experience: Nothing too serious, some interesting semester long projects in economics, scientific computing, and machine learning as an undergrad, MS thesis in progress on multigrid methods for constrained PDEs
Awards/Honors/Recognitions: top departmental award for math as an undergrad (in a small but talented class), honorable mention in the ICM, graduated with distinction in economics
Pertinent Activities or Jobs: working as TA, grader, and tutor for the past two years
Any Miscellaneous Points that Might Help: I know my recommenders pretty well, including one that advised me on several projects as an undergrad (and my MS advisor obviously). Seems like they might write strong letters. Two are graduates from schools I am applying to (doubt that matters though). I only started taking upper level math courses as a senior undergrad and did very well compared with more experienced and talented students. This will show up in my LORs. Half my MS course grades have been A+, rest A's, cumulative upper-level math GPA is 3.97 (one A-). On the other hand, the grad courses were pretty hit-or-miss in terms of content and pacing. Included in my SOP a link to my GitHub page which contains codes that I've written as part of my thesis.

Reach:
Ohio State (Pure) Waitlisted 3/6 -- rank in the range 15-20 of 66 on the waitlist; 3/31 rank updated to top 10;Rejected 4/17
UIUC (Pure, but expressed interest in CSE program in SOP) Waitlisted 2/27; Rejected 4/13
UC Irvine (Pure) Waitlisted 3/16 after inquiry to Prof Guidotti, his response: "...you are still on the waitlist but not ranked very high....unclear whether it will come to an offer of admission at this point"; Rejected 4/13
Penn State (Pure) Rejected 3/6
Boston University (Pure) Waitlisted 3/16 after email to DGS. No further info; Accepted 3/19 via unofficial email from DGS, official offer to come :); 3/23 received official offer of ~34k/year teaching fellowship (summers included) + student health insurance + tuition;
University of Washington (Pure) Rejected 3/2
UCSD (CSME) Rejected 3/6
Match:
LSU (Pure) [did not submit MGRE] Admitted 2/13 w/ $20000 Grad Assistantship + $3000 Graduate School Enrichment Award (4 years) + $550 for health insurance + tuition; 3/29 offered $5k additional funding for first year; offered $30k/year 4 year fellowship 4/9; Declined 4/15
UGA (Pure)

Really nervous I won't get in anywhere at all :o

2/14 One acceptance so far. Nothing from the other schools, not feeling confident :/

2/27 Gah! Really sad about being waitlisted by UIUC. Couldn't expect much more, I suppose

3/6 o sad sad day.

3/15 My guess right now is that I'm rejected from all the schools I haven't heard from. This leaves me with Ohio State being my only remaining (realistic) chance of acceptance, although I think that may unlikely as well.

3/16 Waitlisted at a couple schools, although my chances at those don't seem particularly good either.

Undergrad Institution: Medium sized, liberal arts, public state school
Major(s): Mathematics
Minor(s):
GPA: 3.45
Type of Student: domestic white male

GRE Revised General Test:
Q: 163 (84%)
V: 162 (91%)
W: 4.0 (60%)
GRE Subject Test in Mathematics:
M: 680(55%)

TOEFL Score:

Program Applying: Pure Math

Research Experience: Small, ongoing research project with one professor.
Awards/Honors/Recognitions: Nothing to write home about.
Pertinent Activities or Jobs: Private tutor for a few years.
Any Miscellaneous Points that Might Help: I'm sure that my letters are super strong.
Any Other Info That Shows Up On Your App and Might Matter: Untraditional student. Went to college for two years at 17 years old, got bored and dropped out, spent about ten years pursuing a different career, went back to school to earn my undergraduate degree and study math in earnest. After graduating I've spent the last three years sitting in on undergraduate classes that I either did not take, or I did take but felt like I missed out on a lot of understanding.

University of Florida --- Rejected (March 7) :(
University of Miami --- No response as of May 1. They took my $85 and ran.

It's quite possible I'll be rejected from all of these, so if I get accepted (with funding!) to at least one school, I'll be thrilled.

Even though I'm a bit late, I figure I'll post this as a few aspects of my application are unusual and so the results might be helpful to some others in a similar boat:

Undergrad Institution: Medium-sized private university, top 20 overall, but only ranked 40-ish for grad math
Major(s): Math
Minor(s):
GPA: 3.91 overall, 3.98 for math (the latter calculated by me, so schools might ignore it)
Type of Student: Domestic White Male
GRE Revised General Test:
Q: 170 (97%)
V: 169 (99%)
W: 4.5 (~80%, don't want to double-check)
GRE Subject Test in Mathematics:
M: 890 (93%)

Program Applying: Math, indicating desire to work in arithmetic geometry

Research Experience: No REU's sadly, research program in math (focusing on some working on algebraic topology/algebraic geometry) at home school after freshman year, direct (funded) research with comp sci professor after junior year; both projects ongoing
Awards/Honors/Recognitions: Putnam top 100 one year (just barely), school research grants, full scholarship to undergrad institution
Pertinent Activities or Jobs: TAing/grading for a couple courses (including first-semester undergrad topology), worked for local, selective high-school math camp as counselor after sophomore and senior years
Any Miscellaneous Points that Might Help: (large number of grad courses, see below)
Any Other Info That Shows Up On Your App and Might Matter:
- Currently taking a gap year, tutoring and continuing work on research projects
- None of my research projects have resulted in a paper so far (probably the biggest hole in my application); tend to bite off more than I can chew as I have high expectations for the work I do being interesting, which I tried to convey in the statement, but probably still very much a negative here for my reach schools at least
- Something like 15 math graduate courses with all A's except one A- (plus two in econ and a bunch in CS, though two of the CS ones are B's, from a bad senior fall, not sure whether this will hurt, but in any case shouldn't hurt too much as it's not math)

Applying to Where: (all pure math)

Reach:
Harvard - Rejected (2/5)
MIT - Pending
Chicago - Rejected (2/8)
Caltech - Pending

Match:
Michigan - Pending
Cornell - Admitted (2/5)
University of Wisconsin--Madison - Admitted (2/9), pending central grad school review, but it seems like this is a formality

Low Match/Safety:
Emory - Pending

Undergrad Institution: Pennsylvania State University
Major(s): (Computational) Mathematics
Minor(s): N/A
GPA: 3.92/4.00
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 167 (92%)
V: 164 (94%)
W: 4.0 (60%)
GRE Subject Test in Mathematics:
M: 810 (81%)

TOEFL Score: N/A

Program Applying: Applied Math Ph.D.

Research Experience:
1 Applied Math REU at Penn State where I researched numerical methods. 1 semester of assisting "Data Mining" research where I wrote Python scripts that gathered data from web-services to later be "mined" by the professor.
Awards/Honors/Recognitions: A couple standard university-wide awards for simply having a high GPA. A couple selective scholarships through the "College of Science" at Penn State.
Pertinent Activities or Jobs: Software Engineering Intern at Microsoft. Tutor for several math and statistics classes. Member/leader of "Nittany Data Labs", a club that does some training and projects in data science/machine learning.
Any Miscellaneous Points that Might Help: None that I can think of.
Any Other Info That Shows Up On Your App and Might Matter:

Applying to Where: (Color use here is welcome)
Carnegie Mellon - Statistics Ph.D. / Rejected (2/9)
Duke - Applied Math Ph.D. / Rejected (3/13)
John Hopkins - Applied Math Ph.D. / Admitted on (3/16)
UC Berkeley - Applied Math Ph.D. / Rejected (2/21)
UC Davis - Applied Math Ph.D. / Admitted on (1/24)
UCLA - Applied Math Ph.D. / Admitted on (1/24) / Attending
UC San Diego - Applied Math Ph.D. / Admitted on (2/7)
University of Michigan - Applied Math Ph.D. / Waitlisted (3/16)

My results have far exceeded my expectations. While my GPA and Subject GRE score were solid, I did not have any stand-out research nor did I take many grad classes. In my estimation, my success was probably due to having strong letters and carefully researching each schools faculty members. I specifically targeted schools that had faculty whose math research overlaps with machine learning, as there are relatively few math researchers working in the area. I was originally planning on working as a software engineer until a couple professors at my school told me that my credentials would give me a chance to study with funding at reputable departments. In my case this did in fact turn out to be true, and I am very thankful that they encouraged me to apply! Best of luck to everyone.

Test scores are not everything. If the rest of your application is strong and if you're clear of what you wanna do with a PhD/ MSc, then you can achieve your goal. I thought UF would be the best fit out of the list of my preferred universities. I tell you what I got exactly what I wanted. I'm thrilled and excited to get things started there.

M: 910 (97%)

Program Applying: Pure Math

Research Experience: One REU in summer 2017, which turned into an independent study. We plan on publishing but not before decisions are made.
Awards/Honors/Recognitions: Won a math competition for freshmen/sophomores two years in a row at my school. Made Dean's list every semester.
Pertinent Activities or Jobs: TA for intro to proofs class starting fall 2017.
Any Miscellaneous Points that Might Help: Several graduate classes.
Any Other Info That Shows Up On Your App and Might Matter: I'm finishing undergrad in 3 years for personal reasons, so only two years of grades show up. My last year I'm doing mostly graduate classes and research, so I'm really hoping schools take that into account.

Undergrad Institution: Small State School (Minnesota)
Major(s): Mathematics
Minor(s): Statistics
GPA: 3.9/4.0
Type of Student: DWM

GRE Revised General Test:
Q: 159 (73%)
V: 154 (65%)
W: 4.0 (60%)
GRE Subject Test in Mathematics:
M: 560 (26%)

TOEFL Score: N.A.

Program Applying: Interested in Applied Math and Statistics Ph.D. programs (so all ''Pure'' programs have applied elements I am interested in)

Research Experience: Independent project at my school, no publications. I have been working on it for about 10 months, so I have become really close with my project advisor (he is one of my letter writers). I also mentioned in my personal statement how this project allowed me to mature in mathematics which is why I am applying to graduate school.
Awards/Honors/Recognitions: Dean's List all semesters, member of PME, "Certified" Peer Tutor (an award given for having a certain number of hours with students).
Pertinent Activities or Jobs: I have been a tutor for the last year and a half; I primarily tutor discrete math, calc I and II, and have helped some students in their intro to proofs course. I also mentioned how this drives me to pursue graduate studies as I enjoy teaching others.
Any Miscellaneous Points that Might Help: Nothing really. I am really close with all of my letter writers.

As a note, I am including my relatively lackluster application on this page to give people like me hope. I have poor test scores (especially for a math student), no significant research experience (publications, etc.), and am from a small, unknown, school. There is hope people! Congrats to everyone on their admissions.

Undergrad Institution: large state school
Major(s): mathematics, statistics
Minor(s): actuarial science, German
GPA: 3.86
Type of Student: Domestic white female, first-generation American

GRE Revised General Test:
Q: 168
V: 161
W: 4.0
GRE Subject Test in Mathematics:
M: 590 (34%) (Whoops. test taking skills.)

TOEFL Score: NA

Program Applying: Pure Math PhD

Research Experience: Top REU program, which produced four papers. One paper published, three preprints on arxiv (update 2/22: one accepted for publication this week). Completed the first undergraduate thesis ever awarded by my university's math department which included original research. Capstone paper. Two presentations and a poster presentation at national conferences, with two awards. Several presentations locally at my university.
Awards/Honors/Recognitions: Full ride merit scholarship to my undergraduate institution, one math merit scholarship, one general merit scholarship. Two awards for conference presentations.
Pertinent Activities or Jobs: Honors multivariable calculus TA, grader for introductory analysis, university math tutor for honors math students.
Any Miscellaneous Points that Might Help: Glowing letters of recommendation. One letter from a well known professor emeritus from another university, who I did research with. One letter from my thesis adviser who also advised a graduate reading course with me. The third letter came from a professor who offered to write me a letter without me even asking for it (although obviously appreciated!). All of my core courses in mathematics were honors courses. Four graduate courses.
Any Other Info That Shows Up On Your App and Might Matter: Had some very bad years with medical issues including a major surgery with about five months recovery where my grades suffered a lot, including 3.0 both semesters of algebra.

Applying to Where:
Stanford - rejected 2/28
UT Austin - never got a response, even after contacting them about my NSF fellowship
Maryland College Park - accepted 4/3 with funding and fellowship
UPenn - rejected April 7
Stony Brook - rejected 3/14; accepted 4/5 with funding
Boston College - no feedback as of 4/17
UC Berkeley - rejected 2/22
Rice - waitlisted 2/23; accepted 4/4 with funding and fellowship
UMass Amherst - accepted 2/15 with funding and fellowship
UChicago - rejected 2/8
UCLA - rejected 2/28
Columbia - rejected 2/5
U Washington - waitlisted 3/12; accepted 4/3 with funding
UCSD - rejected 2/24
UNC Chapel Hill - waitlisted 2/13; accepted 4/3 with funding and extra funding added

Skewed my applications very far towards reach schools at the recommendation of my best-known letter writer, whose letter should carry a bit of weight and whose opinion I trust. A lot of these schools will certainly disregard my application on the basis of math GRE score alone, but I'm doing my best to contact schools and make sure that my application gets read because the rest (and especially my research background) is very strong. Best-case scenario, I get an NSF GRF which convinces a few schools to take a gamble on a quantitatively subpar student. Almost guaranteed I'll be the lowest GRE math score at any institution I attend, so it's an uphill battle. I imagine that a lot of the schools that will accept me won't do so until much later in the applicaiton cycle. I at least have one acceptance in the bag now.

Update 2/22: Berkeley came in with a rejection, which is not surprising. Berkeley, Chicago, and Columbia are all departments where I didn't think I'd be a good fit, but applied in case they thought I was a good fit. Maybe just should have trusted my intuition and saved the time and money on these, but hey. My favorites are still standing, some of which seem to be slow on acceptances right now. I have to send out an update to my remaining schools that another of my papers was accepted to a journal (pending revisions), which will hopefully work in my favor. I'm expecting these last days of February to come with a few more rejections--I'm guessing that if I'm getting rejections then UCLA, Stanford, and UT Austin should be here within the next week and a half. Let's see.

Update 3/12: Collecting a lot of waitlists. Looks like I won't know where I'm going until very close to the April 15 deadline.

Update 4/3: I GOT AN NSF FELLOWSHIP.

Update 4/4: A lot is happening in the fallout of the NSF award. Since my information is identifiable and out of respect for the programs I'm talking to, I'm going to wait until this is squared away to make the final updates of my list. Good things are happening! Nervous and excited to see how it turns out.

Update 4/17: Holy cow, it's finally over. I'm going to Rice! Also I'm having trouble with the formatting because there's a bracket missing somewhere, and I have homework to do but you get it.
Bonus update 4/17: Fixed it.

Undergrad Institution: Big State School (with mid-ranked grad department)
Major(s): Mathematics
GPA: 4.0
Type of Student: DWF

GRE Revised General Test:
Q: >160 (>85%)
V: >165 (>95%)
W: >5 (>90%)
GRE Subject Test in Mathematics:
M: <600 (<35%)

Program Applying: (Pure Math PhD, unless otherwise noted)

Research Experience: One REU after my junior year, two independent study courses with a professor at my school. Paper from REU submitted for publication but not published yet. My REU group gave two talks on our research
Awards/Honors/Recognitions: Dean's list all semesters, scholarship from the math department
Pertinent Activities or Jobs: Tutored at on campus tutoring centers, both for the math department and separately. Was a grader for a linear algebra course. Go to a graduate seminar weekly and have presented in it a couple of times.
Any Miscellaneous Points that Might Help: I'm taking my university's graduate algebra sequence, and have taken a couple of courses that are undergraduate/graduate mixes.
Any Other Info That Shows Up On Your App and Might Matter: I've only had one semester of real analysis and no topology due to scheduling conflicts, which definitely hurts. Also, not having taken analysis, topology or calc IV before the math gre was painful and I bombed it, which I'm sure hurts my chances.

Applying to Where:
UT Austin - Math PhD / Pending
Colorado State University - Math PhD / Admitted with TAship (2/14)
University of Oregon - Math PhD / Admitted with TAship (1/30)
Oregon State - Math PhD / Admitted, funding pending (2/15)
UC Santa Cruz - Masters / Admitted, funding pending (2/16)

A suggestion to everyone here posting their results: please preview your post and see if the colors you chose are easily legible. I've seen some atrocious choices of colors used here in highlighting acceptances/rejections. I'd love to be able to read your posts without having to highlight your text and without getting a headache. Thanks.
