I will warn you, however, that mathematics graduate programs do not pick people with some sort of strict formula. That said, I believe that the years of data that are forming on this site are a good source to see where students might be competitive. Keeping that in mind, please adhere to the following format:

Undergrad Institution: Clemson
Major(s): Physics, Math
Minor(s): Philosophy
GPA: 3.6/4.0
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 161 (81%) Not my best, but I don't care enough to retake.
V: 160 (84%)
W: 4.5 (80%)
GRE Subject Test in Mathematics:
M: 590 (33%) Didn't report obviously.

TOEFL Score: N/A

Program Applying: Applied Math

Research Experience: Did about 8 months of research in atmospheric physics, mostly modeling. No paper submitted yet. Presented in local group seminars, but no conferences or anything.
Awards/Honors/Recognitions: Honors college, departmental honors in physics, cum laude, dean's list, Golden Key, NSCS.
Pertinent Activities or Jobs: Worked as a paid physics research assistant for 8 months (see above).
Any Miscellaneous Points that Might Help: For what little it might be worth, I'm an army veteran with service in Afghanistan. I was also chapter president of my fraternity.
Any Other Info That Shows Up On Your App and Might Matter: I graduated undergrad last December, so I have my degrees. That may hurt, help, or not matter.

Applying to Where: (Color use here is welcome)

Reach:
1) Miami - accepted 3/18 (funding: none) Declined
2) NC State - rejected 4/10
3) Tennessee - accepted 2/3 (funding: none) Declined
4) Florida State - accepted 3/4 (funding: none) Declined

Middle:
1) South Carolina - accepted 4/14 off the waitlist (funding: 17.5k + 4k summers, full tuition, health insurance) Attending

Safety:
1) UAB - accepted 4/3 (funding: 20.8k for 12 months, full tuition, health insurance) Declined
2) Houston - accepted 4/18 (funding: none) Declined
3) UNC Charlotte - accepted 3/11 (funding: 16.8k for 9 months, full tuition, health insurance) Declined

Note: To determine what I considered reach, middle, or safety, I used Peterson's acceptance rate data for these programs. Acceptance of <40% is a reach, of 40-70% is a middle, and >70% is a safety. Needless to say, I'm extra cautious lol.

Followup: In hindsight, using the Peterson's data was probably worse than using rankings. A big problem is the ambiguity of what "accepted" means to each program. Some report everyone accepted, including masters students. Some report only those accepted to the PhD program. Plus, none seem to report how many are accepted with funding, so one could easily get accepted, but not funded (which you can see happened to me a lot). The lesson: Take what you consider reach/middle/safety schools with a grain of salt, though rankings are still probably the best way to guess.

Undergrad Institution: Harvard
Major(s): Math
GPA: 3.93/4.00; Major GPA: 3.97/4.00
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 170 (98%)
V: 170 (99%)
W: 5.0/6.0 (93%)

GRE Subject Test in Mathematics:
M: 770 (75%)

Program Applying: Pure Math: Harmonic Analysis and PDE

Research Experience: Nothing I know to be original. A fair amount of original expository work.
Awards/Honors/Recognitions: graduated cum laude, National Merit Scholar, John Harvard Scholar.
Pertinent Activities or Jobs: Taught linear algebra and real analysis to high school students for a year.
Any Miscellaneous Points that Might Help: Ambitious coursework; did very well in the five graduate courses I took in my last year. I created and maintain a math blog with decent traffic.
Any Other Info That Shows Up On Your App and Might Matter: I graduated a year early in 2012 and was hired by a school to design a course for its top math students. I made the mistake of not having letters of rec written and put on file before I graduated. So letters will be written by people whose last interactions with me were ~2 years ago. Based on my email correspondences with the professors, I think they will be strong; but schools might consider me "unwanted goods" not being fresh out of undergrad.

Undergrad Institution: Arguably in the top 2 in my country
Major(s): Electrical Engineering
Minor(s): Electronic Engineering
GPA: 80%
Type of Student: International Male

Edit: After using the WES GPA calculator to estimate my GPA on the U.S. scale, it converted to about 3.45 of 4.0 https://www.wes.org/calculator/igpa.asp

GRE Revised General Test:
Q: 800 (92%) approx. 170
V: 590 (81%) approx. 156
W: 4.5 (80%)

GRE Subject Test in Mathematics:
M: 560 (26%)
Horrible, horrible, horrible GRE score; but I only sent it to Notre Dame ACMS. I really thought it was going to be better, but I obviously needed more preparation as a non-traditional student. So many unfamiliar math topics, and I have been having to read up since then. A little too late.

Program Applying: Applied and Computational Math (Ph.D.) except for Stanford ICME Masters

Research Experience: Informal research only. Assisted a masters student at Imperial College, London with computational aspects of his petrochemical thesis. Previously assisted a PhD candidate with her reservoir simulation software project while I was an undergraduate.
Awards/Honors/Recognitions:
Pertinent Activities or Jobs: Graduated (BSc) 7 years ago. Been working as a software developer since then.
Any Miscellaneous Points that Might Help:
Any Other Info That Shows Up On Your App and Might Matter: Strong interests in academic track.

Applying to Where: (Color use here is welcome)
KAUST - (no response whatsoever till now. No longer expecting any. Withdrawing.)
Stanford - REJECTED 15-Mar (Besides the obvious Stanford "standards", one of my referees was a serious problem; didn't submit my letter)
Rice - REJECTED 15-Apr-2015
Notre Dame Admitted with funding guaranteed for 5 years. Probably attending, as I have no feedback from the other places I applied to. ACCEPTED. ATTENDING.
U Maryland REJECTED 10-Apr

Any Miscellaneous Points that Might Help: I'm writing a textbook on mathematical thinking, will be working on a MOOC on rational inquiry for the Indian Government, and might be involved in the writing of the national textbooks
Any Other Info That Shows Up On Your App and Might Matter: The problem with my application is that I have taken a three year break since finishing college, which I have heard is harmful to the application. Also, I am not very good at tests with two minutes per question, and calculus was a lot longer ago for me than for most others, so I am not looking forward to my MGRE results!

Applying to Where: Not sure but this is my first list (might be a little optimistic!):
University of Texas
Stanford
Dartmouth

Thoughts:
I wish I knew what dragged my applications down the most. In my opinion, the worst aspects of my profile were my GPA and GRE scores. My undergraduate institution was not well known and my department did not offer graduate degrees, in fact my degree program was very new. I am also changing research interests from Applied Mathematics to Statistics. Although the change is not significant, graduate programs may have preferred a Statistics background instead of my background in PDEs. I know that the NSF reviewers did not like that my research background was so diverse.

On the other hand, the best parts of my profile were the two publications, one in a major journal, and the awards and honours I received from my undergraduate school and department. I had a lot of interaction with my recommenders during my undergraduate years and am certain that their recommendations were very strong. I was not able to continue directly to graduate school after completing my undergraduate work. Instead, I spent the year and a half working as a Data Scientist where I was on the application end of Statistics. I do not believe this was seen as a bad thing.

Although I am surprised with the outcome of my applications, I cannot say that I am disappointed. I am very excited to be attending Johns Hopkins University and am honoured they have given me the opportunity to be a graduate student there.

Undergrad Institution: Foreign (Asia)
Major(s): Mathematics
Minor(s):
GPA: 3.60/4.00
Type of Student: International

Grad Institution: ETH, Zurich
Major(s): Mathematics
Minor(s):
GPA: 5.45/6.00

GRE Revised General Test:
Q: 166 (92%)
V: 164 (93%)
W: 4.5 (80%)
GRE Subject Test in Mathematics:
M: Giving on the 25th of October

TOEFL Score: 118/120

Program Applying: Applied Math

Research Experience: Spent a year as a research assistant in Mathematical Biology at my undergraduate institute. 2 publications in recognised (but not particularly fancy) math-bio journals. Currently doing two projects at my grad institute. The first one is about entropy measure-valued solutions for compressible fluid flow and the second one is about regularity theory for stochastic differential equations. One of these projects will be the subject of my Master's thesis.
Awards/Honors/Recognitions: Dean's List all four years at my undergraduate institute. Graduated with distinction.
Pertinent Activities or Jobs: One semester TA-ship for single-variable calculus.
Any Miscellaneous Points that Might Help: Grad classes I have taken so far include Functional Analysis, Elliptic and Parabolic PDEs, a CFD course, Measure Theory, Probability Theory and Stochastic Ordinary Differential Equations.
Any Other Info That Shows Up On Your App and Might Matter: Nothing much comes to mind. Got awarded a Fulbright Scholarship that I didn't avail. Also, my undergrad institute is almost totally unknown. So having done well at ETH Zurich will reflect well on my application I think.

Applying to Where: I have a tentative list but the final list will depend on what score I get in the MGRE. I have a pretty good idea about what I want to do for my Phd so I will most likely only apply to specific schools that have faculty working on Systems of Hyperbolic Conservation Laws. U of Washington, Seattle and U of Maryland, College Park come to mind. But if I get >80% on the MGRE, I might apply to some of the top ten schools.

Undergrad Institution: Foreign
Major(s): Mathematics (nothing like minor)
GPA: about 3.85 if converted for BSc, will be 4.0 for my master's
Type of Student: international white male

GRE Revised General Test:
Q: 170 (98%)
V: 160 (84%)
W: 4.0 (56%)
GRE Subject Test in Mathematics:
M: 810 (82%)

TOEFL Score: (111 = R30/L29/S24/W28)

Program Applying: Pure Math

Research Experience: none
Awards/Honors/Recognitions: several scholarships
Pertinent Activities or Jobs: long-time tutor
Any Miscellaneous Points that Might Help: several grad classes, as I will have an MSc; will have an MA in Linguistics too; know pretty well what I want to do
Any Other Info That Shows Up On Your App and Might Matter: well-known recommenders

Applying to Where: (all Pure Math except if stated otherwise or there is no distinction between applied/pure)
dreaming:
Berkeley Logic - rejected (3/20)
UCLA - rejected (2/25)
Harvard - rejected (2/20)

a little bit less of dreaming:
Rutgers - unofficially accepted (3/16) with 26k TA for first year; support for other years guaranteed; officially accepted (3/20)
Carnegie Mellon - submitted
UC Irvine - accepted (4/3) with 18k TA

As you see, I don't have any safety options. I know pretty well what field I am interested in and I am applying - more or less - to all universities where research in this area is done. If it doesn't work out I might do other things and apply again next year.

Undergrad Institution: top 10 math school
Major(s): Mathematics
Minor(s: Physics
GPA: 4.9/5.0, 10/156 in class
Type of Student: Domestic Chicano Male

GRE Revised General Test:
Q: 170 (98%)
V: 169 (99%)
W: 4.5 (80%)
GRE Subject Test in Mathematics:
Oct 25: 730 (67%) this is now the weakest aspect of my application. yay me for barely preparing!

Program Applying: PhD in Geometric Analysis/PDE

Research Experience: 4 research projects in undergrad, 2 substantial. One (in applied tropical algebra) resulted in 3 conference papers/invited talks, and most of the results are being incorporated into a book. The other (in non-linear hyperbolic pde) resulted in a preprint, which is currently in preparation.
Awards/Honors/Recognitions: Nothing special. supportee of donor research fund, national merit/hispanic scholar, AP stuff...
Pertinent Activities or Jobs: Besides the paid research positions, lots of teaching. Taught math classes to middle/high schoolers under university program. Have held various math tutoring positions.
Any Miscellaneous Points that Might Help: I think that I will have at least one extremely strong recommendation, and the two others will be great/good. All from very respected faculty in analysis and geometry. Also I will have taken around 14-16 graduate classes in math and physics by the time I graduate, and 13 by the time I've applied.
Any Other Info That Shows Up On Your App and Might Matter: I organize weekly advanced seminars for undergraduates on various math topics.

Applying to Where:

Beyond Reach:

Princeton - Rejected 1/22
U Chicago - Accepted 4/3 - Declined

Reach:

NYU Courant - Accepted 2/5 w/ 5 yr 32k/yr MacCracken Fellowship - Declined
UC Berkeley - Accepted 2/3 w/ 5 yr 30k/yr Chancellor's Fellowship - Attending
UC Los Angeles - Accepted 1/23 w/ 5 yr 27k/yr support - Declined
Cambridge - Withdrawn 2/5. Their funding system for foreign students is a mess! Also at this point I have other choices.
Columbia - Rejected 1/29

Manageable:

Brown - Accepted 2/16 w/ 5 yr 27k/yr support - Declined
ETH Zurich - Received email on 6/11 about scholarship - Declined

Grants:
NSF GRFP

Take Aways: If you f*** up your GRE's it isn't the end of the world (like I thought it was), but I wouldn't recommend it. Also, if you're applying to Oxbridge, spend some time figuring out the funding situation in October or November.

Undergrad Institution: Big state school
Major(s): Math
Minor(s): None
GPA: 3.34 (explained below, but it's really 3.60)
Type of Student: Domestic Asian Male

GRE Revised General Test:
Q: 168 (95%)
V: 162 (89%)
W: 4.0 (56%)
GRE Subject Test in Mathematics:
M: 800 (80%)

Awards/Honors/Recognitions: Putnam top 500 in 2013
Pertinent Activities or Jobs: Was a quant over the summer (not sure how relevant), taught math at a camp
Any Miscellaneous Points that Might Help: I suspect I have one or two extremely strong letters plus one okay one, but this is obviously something I can't say for sure. 2 of 3 recommenders are somewhat famous but I don't know how much that matters. 9 graduate classes, grades not super great (a tiny bit better than overall GPA would suggest) but pretty solid junior year, did a reading course thingy too which I'm getting a letter for.
Any Other Info That Shows Up On Your App and Might Matter: I have an F for a silly reason that the professor agreed to waive (but hasn't gone through with the dean and whatever yet), so my GPA looks horrible, but it's addressed by the professor.

Applying to Where:

Reach:
UC Berkeley Rejected, 3/3/2015
MIT Rejected, 2/11/2015
Stanford Rejected, 1/30/2015

UCLA Rejected, 1/29/2015
Brown Rejected, 2/27/2015

Safety:
Not sure, UT Austin, Rutgers, UIUC, UCSD still thinking about it. Application fees suck :/

So nervous... :(

Undergrad Institution: Domestic large undergrad in the northeast
Major(s): BA - Mathematics
Minor(s): Secondary Education and Business Administration
GPA: 3.22 (meh)
Type of Student: Domestic White Female

Grad Institution: Domestic large grad in the northeast
Major(s): EdM, Curriculum and Teaching (focus on mathematics)
GPA: 3.41 (meh)

GRE Revised General Test:
Q: 163 (86%)
V: 158 (78%)
W: 4.5 (80%)
GRE Subject Test in Mathematics:
M: 560 (26%) Needless to say, this was a major blow. My numbers on paper are substandard at best. I don't know how much this will hurt my applications. Some schools didnt require the subject test, but I sent it anyway. I'm not the best speed test taker, and I barely answered half the questions on the exam. Total bummer.

TOEFL Score: n/a

Program Applying: PhD Pure math, focus on number theory and algebra (but mentioned in all my SOPs that I would happy to be on a masters track first)

Research Experience: none
Awards/Honors/Recognitions: none
Pertinent Activities or Jobs: Taught high school math for 4 years, including a year of calculus, and was a math tutor at my undergrad for almost all undergrad math classes. Currently an adjunct professor at a community college teaching learning math support and working in the math lab tutoring. (I would seriously be a PHENOMENAL TA, so I got that going for me)
Any Miscellaneous Points that Might Help: Great letters of rec from grad and undergrad professors, plus my school principal (I know its not an academic reference, but I worked there for four years and he really knows I know my math stuff, and my teaching stuff too). Being a girl helps too, I think. I'm fine with that, I'll take what I can get.
Any Other Info That Shows Up On Your App and Might Matter: Did the PROMYS for teachers program at BU, intensive graduate course on number theory. It was awesome and I rocked at it.

Applying to Where: Spreading quite a large net (sorry letter writers! :? )

Dreams and Wishes:
University of Washington - Rejected 2/12/15 via email to check status
Columbia University - Rejected 1/29/15 via email to check status
UT Austin -Emailed math department to ask, told they could not make me an offer. 4/8/15
UI Urbana Champaign- Rejected 2/23/15 via email (kind of rudely, really).
Northwestern University-Rejected 2/11/15 via email
University of Michigan-Rejected 3/21/15 via email.

Here's Hoping:
UC Boulder -Rejected 2/25/15 via email.
Ohio State Rejected 4/8, response to my email asking about status
UC Santa Barbara-Rejected 3/3/15 via email.
UC San Diego-Rejected 3/6/15 via email.

In the realm of good possibility:
Boston University - Rejected 4/2/15 via email
Northeastern University-Apparently Rejected Emailed 4/2/15 to check, they never wrote back or got in touch with me in any way to let me know their decision. Whatever Northeastern. They were my undergrad so I figured what the hey, but they don't have the program I want anyway. Good riddance.
Tulane University-Waitlisted 4/6/15 Withdrew application Got an offer from Hawaii first, still on waitlist here, so I withdrew.
George Washington University- Rejected for the PhD program, but admitted to their masters, unfunded. 4/1/15
University of Hawaii at Manoa- Admitted! Admitted 4/2/15, was on short list for TA offer as of 4/8/15, offered TA position 4/10/15! $17.5k stipend, tuition waiver and health insurance. ATTENDING!

Wow, it only takes one. 15 schools, only one acceptance, but I really lucked out - the department seems like an incredible fit for me, I have a few contacts there, and I am absolutely thrilled. Looks like I'm moving to Hawaii!!

Looking at the previous years' threads has been a huge help in applying, and I'm posting this to return the favor!

Undergrad Institution: University of Rochester (small private school)
Major(s): Math, Chemistry
GPA: 3.81
Type of Student: Domestic White Male

GRE Revised General Test:
Q: 170 (98%)
V: 165 (95%)
W: 5.0 (93%)
GRE Subject Test in Mathematics:
M: 710 (63%) (GET REKT LOL)

Program Applying: Applied Math

Research Experience: 1 REU in organic chem (no publications), 1 REU in graph rankings (two publications), 1 REU in chemical physics (one publication). Did 1.5 years at very-high-ranked chemical physics program, but had to drop out due to disinterest along with a terrible sleep disorder that was ravaging my memory and cognitive function. No publications there.
Awards/Honors/Recognitions: Phi Beta Kappa, high distinction, not much else.
Pertinent Activities or Jobs: TA'ed about 6 classes. Active on StackExchange network.
Any Miscellaneous Points that Might Help: Took a reasonable number of grad classes. Very good with Mathematica, okay with Julia/MATLAB/Igor Pro, rudimentary grasp of C/Python/CUDA.
Any Other Info That Shows Up On Your App and Might Matter: Still dealing with sleep disorder (unlikely to ever go away, unfortunately), but pharmaceutical sleep aids have made the issue much less of a show-stopper. Older than most applicants.

Applying to Where:
Portland State University - Applied Math
University of New Mexico Albuquerque - Applied Math
University of Colorado Denver - Applied Math

Portland State University is my top choice at the moment; not sure about my chances, so if anyone has experience with PSU admissions feel free to PM me (it looks like only one other person on this website ever applied to PSU, and they didn't say whether they got in or not).

Undergrad Institution: Small engineering school in Florida
Major(s): Aerospace Engineering, Mathematical Sciences
Master's Major: Applied Mathematics
Minor(s): None
GPA: 4.0
Type of Student: International hispanic male

GRE Revised General Test:
Q: 162 (89%)
V: 162 (83%)
W: 4.5 (80%)
GRE Subject Test in Mathematics:
M: 770 (75%) (low due to complete lack of background in several math topics such as Algebra, Probability, Complex Variables (at the time), Combinatorics, etc. I have a strong background in analysis though)

TOEFL Score: Not required

Program Applying: Programs in Analysis or PDEs

Research Experience: Did undergraduate research for a year, then was graduate mentor during the summer for a math REU at my school, gave presentations at the YMC, ERUMC, SIAM-SEAS, and accepted for a conference talk at the JMM in San Antonio

Awards/Honors/Recognitions: Outstanding Senior of the Year 2014 (campus-wide honor)
Winner of a scholarship from the math department at my school for two years
Winner of a Physics competition at my school
Best Research Project in Mathematics at my school
Honorable Mention at the Asian Pacific Math Olympiad 2010

Pertinent Activities or Jobs: Tutor for 1 and a half year in math, TA for a full semester now, and will be TA for a full year before the Fall 2015.
Any Miscellaneous Points that Might Help: Took 3 math graduate classes as an undergraduate while double majoring (with math as my second major); being recommended very strongly by 5 math professors at my school, at least two of which are well known in the PDE community, and another is the Vicepresident of my school. I'm Latin-American too?

Any Other Info That Shows Up On Your App and Might Matter:

Applying to Where:

MIT Rejected 02/11
NYU Rejected 03/09
Stanford Rejected 01/30
UChicago Rejected 02/27
Texas-Austin Rejected 04/14
Minnesota-Twin Cities Accepted 04/21, fully funded!! Attending!!
UCLA Rejected 02/26
UCBerkeley Rejected 03/04
Cambridge Rejected 01/20
Oxford Interview Accepted 02/21

Do you think I have a chance at some of them? I applied only to what I perceive as some of the strongest programs in Analysis/PDEs because I would prefer to stay at my undergraduate institution to do my Ph.D over other programs. But if I get into any of the above programs with a reasonable assistantship, then I'll go.

Conclusion and Advice for others: I have learned through this application process that the reputation of the undergraduate school matters a lot; far more than what you would normally expect, and unfortunately so. Also, the Mathematics Subject GRE is extremely important if you did not study at a Top of the Top school, so don't treat it lightly! Finally, make sure to apply to a LOT of places! Even 10 schools may not be enough, especially if you come from a smaller school. I have been accepted to two of my dream programs so I am happy, and in the end it seems to have worked in my favor that I did not apply to lower level schools (because otherwise I might have been accepted in one such school and would not have had the chance to be accepted off the waitlist from Minnesota). Thank you MathematicsGRE community for your support!

Undergrad Institution: Small liberal arts college with strong math
Major(s): Math
Minor(s): Many (~10) physics classes, also lots of Chinese classes. But no formal minor per se.
GPA: 3.31 overall, 3.56 for math, 3.79 math above linear algebra (requested by UCLA and a couple other places)
Type of Student: Canadian male

GRE Revised General Test:
Q: 169 (97%)
V: 166 (96%)
W: 5.5 (98%)
GRE Subject Test in Mathematics:
M: 630 (44%) (my college has very few timed exams)

Program Applying: Model theory and probability (mostly separately, but in principle together)

Research Experience: Undergrad honors thesis in nonstandard analysis, post-bac in complex systems, summer seminar (reading, not original research) in number theory, 3 years (!!) of physics REUs at a national lab. (I switched to math very late) Two papers in preparation aside from thesis.
Awards/Honors/Recognitions: None as such. Not big on Putnam &c.
Pertinent Activities or Jobs: Worked for several years in a lab (was pretty high up — getting extra rec from my supervisor), smattering of teaching and tutoring experience.
Any Miscellaneous Points that Might Help: I don't think that many people apply for logic programs? I think two of my letters will be really excellent and one will be perfunctory.
Any Other Info That Shows Up On Your App and Might Matter: I had a very rough sophomore and junior year personally, so my grades are pretty meh. My grades as a senior are excellent though (except for a C in a music class? Go figure.) Oh, I also have decent French — hence the one program in France.

(Guess the ordering used for programs)

PhD Programs:
Berkeley (Logic and Methodology of Science) - Rejected 3/19
UCLA - Rejected 2/21
Wisconsin - Rejected 2/19
Chicago - Rejected 2/27
Illinois Chicago - Rejected 3/16
Illinois Urbana-Champaign - Rejected 2/6
Carnegie Mellon (Pure and Applied Logic, via Math) - Rejected 4/10
Carnegie Mellon (Pure and Applied Logic, via Philosophy) - Accepted 2/6, 20k TA/fellowship — will attend.
NYU - Rejected 3/20, but admitted to unfunded MA program
Berlin Mathematical School - Rejected 1/27
Washington - Rejected 4/1
Minnesota - Withdrawn 2/19
Toronto - Rejected by mail, postmarked 4/22, received 4/28 — wow, so much for the 4/15 convention

Master's Programs:
Waterloo - Rejected 2/4
McGill - Unofficial offer 3/25 from a CS prof with a math courtesy appointment, if funding becomes available — declined
Oxford (Math and Found. Comp. Sci.) - Accepted 3/23, awaiting funding — declined
Paris VII (Logic and Found. Comp. Sci.) - Rejected 2/13 by scholarship committee (PGSM), could still enroll without funding — declined

Notes:
1. (1/25) While I've been filling out these applications over the last few months, my research interests have shifted a small but appreciable amount. Woops. I wouldn't apply for a lot of these programs now, and I dropped several (Cornell, Michigan, Bonn, Paris XI) along the way. Overall a good learning experience. If I get rejected everywhere I actually like, I may apply for a Master's at Technische Universität Darmstadt — it would be unfunded, but the fees are only a few hundred euros a year, they have people doing things that interest me, and their deadline is after the US admissions cycle.

2. (2/6) Extremely pleased to receive an offer from CMU. The PAL program is very cool — it may well be my top choice, at this point. It was funny to get this on the same day as a first-round rejection from UIUC, a place that seemed like a much better option a couple months ago.

3. (4/29) I may not have racked up a zillion acceptances, but I got into a program that's a fantastic fit for me, and I feel like I'll be starting grad school with more energy and focus than I could have brought to the table even a few months ago. I think I should have realized a bit earlier that the work I'm interested in doesn't just happen in math departments! Certainly, if I had know that 4/5 of my offers would all be from programs/professors that are interdisciplinary with philosophy or CS (or outside the US for that matter), I probably could have saved myself a lot of GRE-related heartache. But ah well. (On a different note, found out subsequently that a lot of the state schools I was rejected from are having a harder time summoning funding for international students, these days. Sad to see the American public education system squeezed, but at least there are privately endowed programs for us aliens.) I suppose my advice for next year's applicants would be: think very carefully about what you want to do, do your best to contact professors with your interest beforehand, take the GRE seriously; and, be open-minded enough to look for a match in places where you might not expect it. Good luck!

Undergrad Institution: University of Waterloo (Top math/stats/CS school in Canada)
Major(s): Statistics, Pure Mathematics and joint Combinatorics and Optimization
Minor(s):
GPA: 91.4% (We do percentage grades, but 90%+ converts to an A+)
Type of Student:
White male, US and Canadian citizen

GRE Revised General Test:
Q: 170 (98%)
V: 166 (96%)
W: 5.0 (93%)
GRE Subject Test in Mathematics:
M: 750 (71%) (Only submitted to Stanford)

Program Applying: Statistics, and one CS. Looking to do research in machine learning.

Research Experience: (all through my university, each term is 4 months) In chronological order, 1 full-time research term in abstract harmonic harmonic analysis, 1 part-time term in applying machine learning to HCI, 1 full-time term in applied machine learning. The last term yielded a first author paper that is currently under review, but my supervisor is quite pleased with. All three professors are quite senior.
Awards/Honors/Recognitions: Various small scholarships for grades and fellowships to do above research - nothing major.
Pertinent Activities or Jobs: I've been through the co-op program, so have 24 months of full-time work experience. I've done 8 months modeling derivatives at a major investment bank, 4 months in using machine learning for prop trading at the same bank, and 4 months at a top technology firm (think Facebook, Google, Amazon) as a researcher working on things directly related to what I want to study.
Any Miscellaneous Points that Might Help: I've taken ~33 math (including stat) classes, including 5 PhD-level courses (plus many more cross-listed masters courses). I asked for 4 letters - one from each prof I've done research with, plus my supervisor at the tech firm (who also has a PhD from a top school). My grades have also risen since the start of third year to be around a 95% average.

Applying to Where: (Statistics unless otherwise specified)
UC Berkeley - accepted, February 12
Stanford - rejected, February 6
Washington - accepted, January 16
Columbia - accepted, February 11
Carnegie Mellon - joint statistics and machine learning, and statistics waitlisted, February 25 (withdrew from list immediately)
Chicago accepted, February 9
MIT - computer science rejected, February 26

It would appear that Stanford puts a lot of emphasis on the subject test, and that those schools that recommend submitting the test (Columbia, Washington and Chicago) may still accept you if you don't submit a score. I'm going to my top choice though (Berkeley), so life is good.
